For your convenience, for ease and to hasten the processing of your registration on your first or return visit these forms can be printed and completed prior to your visit in our office. For new patients please print and complete all 4 forms, bring them along to our office or email them directly to us at forms@risserthomas.com
and please have your insurance cards with you. For patients who have not been here in a year or more, complete just the medical history form and bring that form along with your insurance cards along to your visit or email them directly to us at forms@risserthomas.com. It is a mandated medical care requirement that we have a your medical history and medication list updated on each visit, Thank you in advance for assisting us to meet this requirement.
